Jet Airways flight 9W228 inexplicably descended about 5000 feet in Turkish Airspace on the morning of 8.8.2014, endangering the safety of several flights operating through the busy airspace (news clipping enclosed). An audit was conducted by DGCA following this incident, which resulted in several critical findings indicated serious violations in the Operations department of Jet Airways. Specifically, the DGCA team discovered that 131 pilots were utilized for flights after their Proficiency Check had expired. This amounts to operating an aircraft with unqualified pilots and as a consequence, with invalid insurance. Besides suspending licences of two pilots out of these 131, DGCA did not take any action on this serious issue till Kalchakra raised an alarm. After which all of them were grounded and only a warning was issued to them (see box).
